Application Requirements:
- Detailed description of the program design and proposed accomplishments during the grant
period, including organization's target population, constituents and geographic communities,
capabilities of the organization to carry out the proposed project as well as a budget narrative
detailing all planned expenditures, anticipated income and sources of income. (* proposal
narrative not to exceed 3 pages)
- CV or statement of the qualifications of key personnel (can be added to proposal narrative, does
not count toward 3 page limit).
- Projected budget for grant period.
- Current organizational operating budget.
- List of directors and officers of the organization.
- IRS letter indicating tax exempt status and ID number (1st page only listing entity and ID number).
- List of your largest funding sources for the past fiscal year. Include significant Foundation grants,
Federated funds and/or Government grants, if applicable.
